WellMinds MD is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We are committed to maintaining the privacy and security of your Protected Health Information (PHI) as required by federal and Washington State law.
Our full HIPAA Notice of Privacy Practices is provided to all patients as part of the intake process and is available upon request. It describes in detail your rights regarding your health information and how we may use and disclose it.
Electronic Health Records
All patient records are maintained in a HIPAA-compliant electronic health record (EHR) and practice management platform. We use industry-standard encryption and security measures to protect your data. You can access your records, complete forms, and communicate securely with our practice through the patient portal.
Website & Contact Form Data
This website does not use tracking cookies or sell any user data to third parties. When you submit the contact form on this website, your information is transmitted securely to our practice inbox. We use it solely to respond to your inquiry. Form submission data is not stored beyond what is necessary to fulfill your request.
This site is hosted on Microsoft Azure. Standard server logs (IP address, browser type, pages visited) may be retained for security and diagnostic purposes per Microsoft Azure's standard data retention practices.
How We Use Your Information
Information you provide to us — whether through the contact form, by phone, or during treatment — is used to:
- Schedule and provide clinical care
- Respond to your questions and requests
- Process billing and insurance claims (where applicable)
- Comply with legal and regulatory requirements

Minors
For patients under the age of 18, consent to treatment is provided by a parent or legal guardian. Health information for minor patients is protected in accordance with HIPAA and applicable Washington State law, which in certain circumstances grants adolescents independent privacy rights.
