Emergencies
​For life-threatening emergencies, please call 911 or visit your nearest emergency room. If you message me over the EMR, it may take 24-48 hours for a response. For urgent situations, call the crisis line 24/7 at 360.586.2800, or the National Suicide Prevention Lifeline at 1-800-273-TALK (8255).
Intake Form Policy
All patients must complete the intake package, 24 hours before the initial evaluation. Failure to do so may result in appointment cancellation or rescheduling and a $200 cancellation fee.
Cancellations
Cancellations must be submitted at least 24 business hours in advance for appointments scheduled on weekdays (Monday through Friday, excluding holidays). For example, if your appointment is on a Monday, cancellations must be made by the same hour on preceding Friday. Late cancellations and missed appointments will incur a fee amounting to50% of the appointment cost. Repeated instances of no-shows or late cancellations may result in the discontinuation of treatment to ensure equitable and safe care for all patients.
Payment Information
Our practice is IN-NETWORK with Aetna. We are "out of network" with all other major insurance companies. For "out of network" patients, full payment is due at the time of service by credit or debit card, even if you plan to request reimbursement from your insurance, FSA, or HSA. We use Simple Practice EHR, which ensures HIPAA compliance and maintains the confidentiality of our therapeutic relationship.
Administrative Time Based Fees
To maintain low overhead and provide optimal value, a $400/hour fee is charged for administrative work exceeding 10 minutes outside of appointment time. This includes phone calls, emails, photocopying, document preparation, medical record handling, and obtaining prior authorizations. Time is billed in 15-minute increments. We will always provide a time estimate and seek your approval before performing additional services that incur charges.
Policy regarding Controlled Substances and Prescription History
We review the complete prescription history for all patients, including controlled substances, as required by the DEA and most states. Refills for controlled substances always require an appointment. Please refer to the FAQ for details re: this. NOTE :If you lose a controlled substance medication, ONE refill will be provided; subsequent losses will not be refilled.
